Histamine plays a key role in allergic responses by binding to H1 receptors, which triggers the release of inflammatory mediators such as interleukins, resulting in symptoms like itchiness, nasal discharge, and watery eyes.
Fexofenadine acts as an inverse agonist of the H1 receptor, stabilizing its inactive form and thereby preventing histamine-induced symptoms. It selectively targets peripheral H1 receptors without affecting dopaminergic, serotonergic, cholinergic, or adrenergic systems. Importantly, it does not cross the blood-brain barrier, minimizing the risk of sedation or CNS effects.
Symptoms may include dizziness, drowsiness, fatigue, and dry mouth. High doses (up to 800 mg) have not shown serious adverse effects in trials. Supportive care is recommended, as hemodialysis does not significantly eliminate the drug.

Fexofenadine is not metabolized by the liver, reducing potential drug interactions. Concurrent use with erythromycin or ketoconazole may elevate fexofenadine plasma levels 2–3 times without QT prolongation or additional side effects. No interaction with omeprazole was observed. However, antacids containing aluminum or magnesium may lower absorption; separate administration by at least 2 hours is recommended.
